Measurement of Nursing hour of Nursing Care Activities

Abstract
|

|
Measurement of nursing hour for direct & indirect nursing activities and personal time to establish the standard of nursing time was performed by 19 nursing units and shifts in a university hospital from June 11 to 18, 1990.
To calculate the nursing hours for nursing care activities, the self-recorded performance sheets recorded by 156 staff nurses for 5 days were collected and studied. The results were as follows
1) The total amount of time spent on nursing activities recorded 27.02 hours in MICU and 21.33 hours in delivery room among 6 east units, 6 west units and 7 special units (Oncology, MICU, SICU, NSICU, CCU, Neonatal room, Delivery room).
2) In MICU, 1 east unit of 5th floor and 1 west unit of 4th floor nurses overworked as compared with 8 hours of duty. The greater part of personal time was spent for meal time without other activities.
3) The ratio of direct and indirect nursing hour was 1 : 1.36 in east unit, 1 : 1.55 in west unit and 1 : 1.2 in oncology unit. The percentage of direct nursing activities showed greater than 56 percent in 3 ICUS(MICU, SICU, NSICU). The ratio of direct and indirect nursing hour in CCU was 1 : 0.95, and 1 : 1.24 in Delivery room.
4) The activities of observation /measurement, medication, and giving & receiving assignment of direct nursing activities occupied higher rate of direct nursing hour as comdared with other activities. Recording given cares was main function in indirect nursing activities.
5) In ICUs and neonatal room, the nurses spent higher time in general physical cares of direct nursing activities as compared with those of other nursing units.
6) Non-nursing activities were often sampling & testing and vicarious execution for Dr´s activity (e.g. writing out slip).
